If i'm not mistaken, it was a spot "Gone Wrong". In August 2008 on Monday Night Raw, Chris Jericho had Michaels as a guest on his segment show, The Highlight Reel, lost his temper, attacked Michaels and threw him face-first into the glass of the Jeri-Tron 6000 (the monitor that once was in the background of Chris Jericho's show).
It was supposed to be just a regular spot, but went awry and actually injured Michaels' eye, He suffered a partially detached retina which when healed left him with a "lazy eye".
