As to why Casillas was dropped by Mourinho in the first place, nobody knows for sure, but it's believed that it was because of something that happened behind the scenes between them. It was widely reported that Mourinho only spoke to a small group of the Madrid players when he was there anyway, Casillias wasn't one of them it seems. He was questioned why he kept playing Lopez when Casillas returned, and he simply said that he preferred Lopez.
Not sure why Ancelotti's decided to continue with Casillas on the bench though.
In all fairness, Lopez played out of his skin when he initially came back to Madrid last January, but let's be realistic, he's not Casillas.
If Casillas continues to be favored by Del Bosque though (at the minute, he is still in the team), then he should easily beat 200 caps as long as whoever's in charge keeps playing him.
There's a lot of rumors that Casillas might leave Madrid in January though, so we'll see.
