the neilsen rating system is a joke, it's been broken for years now, and the advent of the cable box did little to fix it.
It is supposed to be a measurement of what televisions are tuned to what program, however only like 65% of televisions have the required equipment to be tracked.

the Content rating system is slightly less fucked, however there are few solid rules when it comes to setting ratings, and a lot of it comes down to who the members on the board like, or what the "overall message" of a show is deemed to be

it means basically nothing, since there is no hard data of how many systems actually report to the neilsen ratings.
Without actual numbers on what percentage of homes are capable of being included in the ratings system, the ratings themselves are completely useless.
